Send Dental Records

Email us at vision@periopeak.com your dental records. It is important the records be sent by the patient via the patients e-mail.

We will need:

A Full Set of X-Rays
A Periodontal Charting (gum measurements)

We only accept digital records.

Step 2

Consultation - $100

Once your records are received and carefully reviewed we will email you to set up a zoom or phone consultation.

We will gladly explain the process or answer any questions you may have.
